Base pay range $34.00/hr - $34.00/hr

Major Responsibilities
Assist in implementing individualized treatment plans for assigned residents.
Supervise CNAs and participate in their evaluation.
Oversee Falls Program and Pressure Sore Program.
Provide nursing services.
Conduct and document thorough evaluations of each resident’s medical status upon admission and throughout treatment.
Other duties as assigned.
Qualifications
Must possess a current, unencumbered, active state license to practice as an LPN.
Experience in skilled nursing or rehabilitation facilities preferred.
