Registered Nurse (RN), Home Health – $15,000 Sign‑On Bonus Available

Responsibilities
Provide services in accordance with the plan of care.
Conduct initial and regular evaluation visits and reevaluate nursing needs.
Initiate and revise plans of care as needed.
Deliver specialized nursing services, preventive and rehabilitative procedures.
Prepare clinical and progress notes, care conference summaries, and maintain documentation per agency policy.
Coordinate services and inform personnel of patient condition changes.
Counsel patients, families, and significant others regarding nursing and related needs.
Participate in and present in‑service programs.
Adhere to agency policies and procedures.
Process orders and notify physicians of patient needs and changes; complete certification/recertification orders and discharge summaries.
Determine nursing requirements for each patient.
Refer patients to PT, SLP, OT, or LCSW as appropriate.
Supervise and teach other nursing personnel.
Conduct patient care conferences.
Participate in peer review, performance improvement, and utilization review.
Perform on‑call duties nights, weekends, and holidays as assigned.
Complete and submit OASIS assessments, reassessments, transfers, resumptions, discharges, and significant change in condition in accordance with agency time frames.
Appropriately utilize ICD‑9 coding.
Qualifications
Graduate of an approved nursing school and currently licensed in the state(s) of practice.
Minimum of two (2) years of nursing experience (preferred).
Acceptance of the agency’s philosophy and goals.
Ability to exercise initiative and independent judgment.
Physical Requirements
Perform physical activity, including standing and walking for extended periods.
Lift, bend, and assist patients regularly; heavy lifting.
