About the Opportunity

We are seeking a full‑time

Lead Care Manager

to play a critical role in our interdisciplinary care model. The position primarily allows remote work, with 2–3 on‑site visits per month in the community alongside a PairMate.

Responsibilities

Maintain a caseload of individuals with serious mental illness, substance use disorder, homelessness, or high medical needs using evidence‑based engagement approaches. Use relationship‑based strategies to support members with social support navigation. Conduct periodic telephonic and SMS outreach to ensure timely follow‑up. Collaborate with the care team through systematic case reviews and consults before taking clinical actions. Meet monthly encounter metrics to comply with health plan regulations. Assist individuals in securing community supports and coordinate physical care appointments.

What You’ll Need

1+ year of general work experience (case management preferred). Residency in California with reliable transportation for engagement at clinic and community locations. Bilingual (English/Spanish) with strong cultural fluency. High degree of empathy, organizational skills, and ability to remain patient when faced with adversity. Strong technical skills and comfort with technology innovation, past experience with CRM databases, basic Excel, Word, email, and video conferencing. Valid driver's license and auto liability insurance.

Preferred Qualifications

2+ years of case management experience. Experience with motivational interviewing and medical terminology knowledge. Detail‑oriented, organized self‑starter who is a rockstar multitasker. Preferred lived experience working closely with individuals experiencing complex chronic needs.
