About the Position

This position is within the Behavioral Health/Mental Health Directorate located at Naval Hospital Oak Harbor, Washington. This is a Direct Hire Solicitation Duties

Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients. Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions. Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care. Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements. Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care. Requirements

Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. A Personnel Security Investigation may be required. Appointment to this position is subject to a one year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 315. While probationary the incumbent can be terminated with limited appeal rights. Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series. This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination. Must be able to obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles This position has been designated "Mission Essential". In the event of severe weather conditions or other such emergency type situations the incumbent is required to report to work or remain at work as scheduled to support mission operations. Licensed as Licensed Master Social Worker or Nurse is preferred.
