Travel between assessment locations will be reimbursed. This includes mileage for personal vehicle use as well as costs associated with public transportation (e.g., subway, bus, train). What You’ll Do
Conduct in-person CAS assessments in homes and community-based settings. Travel throughout the Wayne/Ontario/Seneca to meet with individuals, families, and caregivers. Use a person-centered approach to assess needs, strengths, and support requirements. Interview caregivers, service providers, and other stakeholders to gather accurate information. Review relevant records and incorporate multiple data sources to support assessment coding. Enter data into state-mandated systems and ensure timely, accurate documentation. Maintain productivity and quality standards as outlined by the program. Coordinate interpreter services as needed for non-English/Spanish-speaking participants. What You Need
Education: Required:
Bachelor’s degree in a human services field Preferred:
Master’s degree in a related field Experience: Minimum of 1 year of direct experience working with adults with IDD (required) Experience conducting assessments and prior CAS experience (preferred) Strong analytical, communication, and problem-solving skills Ability to work independently and manage deadlines effectively Proficient in Microsoft Word, email, and web-based systems Bilingual (English/Spanish) strongly preferred Other Requirements: Valid NYS Driver’s License or NYS ID card Willingness to travel within the assigned region Choose 1st Choice — where your compassion meets opportunity.
