We’re looking for a compassionate, organized, and team-oriented individual to join our Home Health team as a
Home Health Aide (HHA)
(Full-Time or Part-Time per diem). As an HHA, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff to provide support both remotely and independently in the field.

Responsibilities
Deliver personalized care and hygiene support to patients in the comfort of their own homes.
Promote health, prevent disease, and care for patients at home.
Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Home Health Aide (HHA) license to practice in California – required.
Experience in Home Health or Hospice care – 1 year required.
Current California driver's license.
Able to travel within a 15-25 mile radius on a regular basis.
