Overview PHS is a faith‑based nonprofit providing care for older adults in various settings. The role is based at Waverly Gardens (5919 Centerville Road, North Oaks, MN 55127) – a 10‑acre community offering Assisted Living, Memory Care, and Long‑Term Care facilities. The facility is located 15 miles north of St. Paul and offers amenities such as off‑street parking, discounted meals, fitness center access, and employee recognition events.

Responsibilities You will provide high‑quality clinical care and monitor the work of Medication Assistants, Resident Assistants, and Resident Aides. You will report directly to the RN, Clinical Coordinator, or Clinical Administrator, and may receive direction from the Community Coordinator. Your goal is to put “Smiles in the Eyes of our Residents” while meeting regulatory and clinical best‑practice standards.

Qualifications

Licensed Practical Nurse with current licensure in the state(s) of practice.

Current CPR certification.

Interest in gerontological nursing and rehabilitative care.

Alignment with PHS’s mission and operating philosophies.

Proficient English communication skills (reading, writing, speaking).

Pay Range Estimated starting pay: $30.76 – $32.99 per hour, based on qualifications and market conditions.
