Compensation
$23/hr – $26/hr commensurate with experience
Benefits Three comprehensive healthcare plans available. Housing Works covers most of the plan; you only pay a portion based on your salary. PTO accrues immediately up to 30 days in the first year. An educational benefit is offered for tuition loan reimbursement, tuition costs, and textbooks.
Overview In partnership with the Mayor’s Office of Criminal Justice (MOCJ), Housing Works Justice Initiatives is launching the CREATE (Cannabis Reentry Education And Training Experience) pilot program. This eight‑week, paid training program includes two weeks of classroom instruction followed by six weeks of on‑the‑job training in an NYC‑licensed cannabis dispensary. The Case Manager reports to the Program Coordinator and is responsible for outreach, recruitment, enrollment screening, service‑plan development, benefits enrollment, and connecting participants to wraparound health and support services.
Responsibilities
Provide case management services to actively enrolled clients.
Participate in outreach and recruitment activities.
Conduct initial assessments to gauge interest and program eligibility.
Perform comprehensive intake assessments with newly enrolled clients.
Assist clients with benefits and entitlements enrollment and vital‑documentation acquisition.
Connect clients to healthcare services such as primary care, mental health, and substance‑use treatment.
Co‑develop treatment plans and review with clients every 60 days.
Facilitate work‑readiness or soft‑skills workshops.
Conduct regular, consistent case‑management sessions with assigned caseload.
Participate in case conferences and other coordination activities.
Engage in crisis intervention as needed.
Document progress notes and client data in the database.
Attend individual supervision sessions with the assigned supervisor.
Promote Housing Works core values in daily activities.
Carry out any additional tasks assigned by the supervisor.
Secondary Responsibilities
Provide culturally competent services in line with Housing Works policies.
Work with clients who have diverse needs ranging in complexity.
Collaborate with team members within the Justice Initiatives Department and other agencies.
Requirements
High School Diploma or equivalent (required). Bachelor’s Degree in a related field preferred.
Minimum one year of case‑management or direct‑care experience.
Experience working with individuals or communities impacted by the criminal legal system.
Knowledge of NYC support services and federal, state, and city benefits.
Strong written documentation and communication skills.
Problem‑solving abilities and capacity to work autonomously.
Proficiency with Microsoft Office (Excel).
Experience with electronic medical record (EMR) systems.
Preference for bilingual Spanish speakers.
Dependable attendance and punctuality for scheduled shifts.
Willingness to travel within boroughs via public transportation.
