Responsibilities
Assess and monitor the physical, emotional, and psychological needs of patients
Create home health care plans that align with MD orders and patient goals
Administer medications, treatments, and interventions
Provide pain and symptom management
Educate and support patient families and caregivers
Collaborate with an interdisciplinary team
Maintain accurate and timely documentation
Participate in on‑call rotation as required by the local branch
Qualifications
Registered Nursing Degree (Associate or Bachelor) from an accredited college with current unrestricted registration and license in the applicable state
Minimum of one year nursing practice in a patient‑care setting; experience in home health or geriatrics preferred
Valid driver’s license, acceptable driving record, and proof of car insurance per company policy
New nursing graduates may be considered in select markets based on program availability
Current CPR certification
