Bachelor's Degree: $58,083 - $69,700

Where will I work?

This role offers a hybrid schedule with work in clients’ homes in San Francisco County, at GGRC offices, and from home.

What you’ll do Lead the needs assessment of applicants (3 years and older) and their families and develop Individual Program Plans for eligible applicants.

Responsibilities

Initial Applicant Assessment

Meets with the applicant/family/guardian within 15 working days and reviews submitted materials

Obtain the completed application form from applicant and/or family (or assist in completion of form)

Clarify the request for services and the needs of applicant/family

Review and analyze data; identify barriers and opportunities in applicants’ development objectives and plans

Provide information regarding GGRC and services available

Interview to obtain social, emotional, developmental, and familial information for social diagnosis

Work with applicants to assess needs, preferences, and life choices

Confirm any immediate needs and provide referrals for necessary interventions or services

Complete Social Assessment report

Schedule and coordinate meetings with team and applicants

Obtain information to complete Applicant Developmental Evaluation Report (CDER)

Obtain signed Release of Information forms for additional data

Submit completed application packet and Releases of Information to Office Intake

Coordinate assessment team completion of the CDER at eligibility determination

Provide information and referrals for applicants not eligible for agency services

Research available community resources and services

Ensure services are provided per the Lanterman Act and Title XVII

Obtain or collect information for use by interdisciplinary team

Provide intake services as needed

Provide crises management as needed

Review and ensure applicants receive appropriate services from existing community resources

Empower applicants and families to participate in coordinating and utilizing resources

Advocate for applicants in unique circumstances

Provide outreach services as assigned

Maintain records and prepare written/verbal reports and miscellaneous paperwork

Participate in staff and internal meetings, and community meetings

Assist community resources in improving services to the disabled; identify needs or gaps

Participate in special projects and other duties as assigned

Maintain all applicant information as confidential in compliance with HIPAA and the Lanterman Act

Requirements Who You Are:

Interviewing techniques to ensure person-centered planning

Knowledge of counseling principles, objectives, and techniques

Knowledge of normal and abnormal behavior, human growth, and family development

Ability to maintain case records clearly and concisely

Written communication: clear, professional business writing

Oral communication: ability to convey complex information clearly

Ability to operate basic office equipment

Basic familiarity with MS Word, MS Excel, MS PowerPoint, MS Outlook or similar

Fluency in Mandarin and Cantonese preferred

Education/Training

Bachelor’s degree in social work or related field required

Local travel within San Francisco, San Mateo, and Marin counties

Occasional statewide travel to sites throughout California
