Skies HC and Rehab Center is seeking a dependable and skilled Licensed Practical Nurse to provide direct patient care on a part‑time contract basis. The LPN will assist in delivering high‑quality nursing care in a rehabilitation or long‑term care setting. Responsibilities:
Provide direct nursing care in accordance with facility policies and state regulations. Administer medications and treatments accurately and safely. Monitor patient conditions and report changes to supervising RN or provider. Maintain accurate documentation of all care provided. Collaborate with interdisciplinary team members to support patient outcomes. Requirements:
Active LPN license in the state of New Mexico. Must bring a state or federal‑issued identification to the facility and wear a photo badge at all times. Must have completed the original or updated COVID‑19 vaccine
or
provide documented religious or medical exemption; additional doses may be required by law. Prior experience in rehabilitation or skilled nursing preferred.
