HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER (Covering Onslow and surrounding counties)
Job Description
Implements palliative care program requirements
Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
Conducts face‑to‑face encounters to provide re‑certification information to Medical Director/Associate Medical Director for Hospice Patients
Provides clinical findings of that face‑to‑face encounter to Primary care physician and/or supervising Medical Director/Associate Medical Director
Communicates with patients/families at all levels.
Communicates with all agency personnel
Job Requirements
Master’s degree in Nursing required, with current licensure in the state of practice
Certification as an Adult or Family Nurse Practitioner
Experience in Hospice or Palliative Care nursing preferred
Knowledge of the framework, function, and organization of a hospice care program.
Excellent communication and organization skills.
Background checks/drug‑free workplace.
