Full Time Hospice RN
We are a growing hospice agency seeking a compassionate, dynamic, and organized registered nurse to provide one‑on‑one care in the patient’s familiar surroundings. The role is based in Troy, MI serving patients in Jackson, MI and surrounding areas. Your responsibilities will include: Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs Implementing and revising the individualized plan of care Complying with AdvisaCare Hospice policies, procedures, and Medicare requirements Consulting with and educating patients/families on disease process, self‑care techniques, end‑of‑life care, and ethical concerns Supervising paraprofessionals providing patient services Preparing clinical and progress notes; participating in IDG conferences Participating in hospice performance improvement programs Maintaining high customer satisfaction among clients, families, and caregivers Communicating information using current processes and technology within the organization Requirements
Current Michigan RN license in good standing Minimum 1 year of hospice experience Empathetic approach to patients and caregivers Participation in daily field visits and weekly office meetings as indicated Ability to adapt to patients’ status and needs on a full‑time, on‑call basis Benefits
