Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. is seeking a

Hospice Licensed Practical Nurse

to care for and serve our patients and their families in our Inpatient Unit located in Wexford.

The inpatient LPN is a full time position working (3) - 12 hour shifts (must be able to work daylight and overnight shifts) and includes a weekend and holiday rotation.

POSITION SUMMARY The Licensed Practical Nurse provides skilled nursing care in the inpatient hospice facility to hospice patients in accordance with the individualized plan of care developed by the patient and/or family, physician and hospice team.

QUALIFICATIONS

Current LPN license in the state of Pennsylvania

Minimum of two [2] years experience in patient care preferred

Demonstrates excellent clinical skills

Excellent organizational skills with attention to detail

Outstanding communication skills (both written and verbal)

RESPONSIBILITIES

Works with the Interdisciplinary Group (IDG) to develop, coordinate, and update the individualized plan of care for hospice patients/families cooperatively with the patient and/or family, physician and hospice team.

Conducts a physical examination and nursing assessment of the patient on each shift and as necessary.

Provides personal care such as (but not limited to): bathing, grooming, dressing, shaving, feeding; assessing vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation.

Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death.

Completes all computer and/or paper documentation requirements within the timeframes specified.

Works shifts as needed, including weekend and holiday rotations.
