is seeking a full-time
Integrated Care Coordinator RN
to engage with behavioral healthcare clients to ensure clients are receiving the most effective and appropriate care in managing their behavioral health and primary care needs.
This position will be Hybrid with rotation in our clinics and eventually two days from home. The hours may vary but are typically Monday through Friday 8:30 to 5 pm.
PERKS & PAY
Salary:
$52,104 (RN) with up to 12% annual bonus
Hybrid schedule rotating between our clinics and eventually moving to some care coordination at home. (Training will include M-F 8:30 to 5 pm on site for up to 6 months)

QUALIFICATIONS
An active RN license in Kentucky is required.
Relevant knowledge, training, and experience.
Excellent written and verbal communication skills.
Psych nursing is preferred but not required.
JOB DUTIES
Engage child, adolescent, and adult clients with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), and substance use disorder (SUD) diagnoses in a structured, collaborative relationship that empowers each client to gain the necessary skills in a variety of areas related to recovery and self‑management of their mental health, substance abuse, and developmental disabilities.
Promote environmental and psycho‑social health and maximize independence to improve and maintain lifelong well‑being and functional status.
Provide evidence‑based care concerning complex cases of vulnerable individuals with SMI, SED, SUD, and/or developmental/intellectual disabilities.
Identify and address risks and current or potential gaps in care to prevent unnecessary deterioration that could result in hospitalizations, emergency room visits, and/or loss of independence in vocational, academic, and/or social functioning.
Provide education to clients to promote improved functioning skills and development.
Complete requests for prior authorizations as indicated to provide necessary services.
Participate in staff meetings and consultation with other clinicians to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services.
Act as an advocate to clients to ensure they receive necessary services in the community.
Develop knowledge and establish good working relationships with community support resources and services within the community.
Maintain communication and positive multi‑disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
Complete timely and accurate documentation within an electronic health record and other documentation as required of the position.
Attend and participate in relevant continuing education and in‑service training to improve professional competency and clinical intervention skills.
Maintain flexible hours to meet agency and client needs.
