The Care Coordinator plays a vital role in transforming the client experience within our outpatient behavioral health services. This position is at the heart of our care team, ensuring that every client receives seamless, well‑coordinated, and compassionate support. Through close collaboration with providers, clinicians, and community partners, the Care Coordinator helps clients navigate their treatment journey with clarity, confidence, and continuity.
In this role, you will facilitate Just in Time scheduling, maintain strong communication across the care team, and connect clients with essential internal and external resources. You will serve as a trusted liaison between behavioral health services and community agencies, helping bridge gaps and streamline care.
Your responsibilities include reviewing records for required documentation, sharing critical updates across systems, coordinating inpatient discharge planning, and supporting specialty care referrals. You will also champion the use of Health Information Technology, document effectively within the EMR to support wraparound care, and contribute to quality management through timely reporting and audits.
A patient‑centered mindset, strong problem‑solving skills, and the ability to manage multiple priorities independently are essential to success in this role.
Qualifications Educational Requirements:
A BA or BS degree or higher in Health and Human Services, Psychology, or a related field.
Experience & Qualifications:
Ideal candidates bring 3–5 years of experience working with behavioral health clients, strong organizational and time‑management skills, excellent documentation and verbal communication abilities, and proficiency in Word, Excel, and Outlook. Most importantly, you bring a collaborative spirit and the ability to work effectively with staff, visitors, and clients from all backgrounds.
