All Metro Health Care, a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor- Home Care.
You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.
Pay per Visit
Sup/Post Hospital: $60
Reassessment: $90
Initial Admission: $120 weekday / $180 weekend
Visiting Client Territory:
Bronx
Required Availability:
Ability to perform visits
after 8:00 PM , as we service patients from
8 PM–8 AM .
Availability for
at least one weekend per month .
Schedule:
Visits planned with the required presence of the patient’s aide.

You will...
Performs on-site supervisory and teaching visits to evaluate caregivers
Provides clinical orientation to caregivers on assigned home care cases.
Participates in the training of new nurses and in-services for field staff.
Performs initial nursing assessments
Counsels patient/caregiver in meeting nursing and related needs.
Assigns and coordinates services provided.
Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
Prepares clinical progress notes ensuring any changes in the patient’s condition is reported
Monitors clinical records for complete and accurate documentation.

1 year of supervisory or management experience required
Valid NYS Driver’s License, insurance, registration, and reliable car
