Position Summary
Under the supervision of unit leadership and the charge nurse, a Licensed Practical Nurse develops and implements individualized nursing care plans to meet residents’ needs, assists in orienting and supervising nursing personnel, and attends to the unit’s daily operations on a per‑shift basis, assuming leadership responsibilities as needed.
Essential Functions
Collects data, conducts ongoing reassessment, and gathers information relevant to the individual needs of patients or families.
Reviews, updates, and follows the initial plan of care created by the RN, ensuring it meets policy requirements.
Implements, documents, and provides patient care and education based on the plan of care.
Evaluates and documents patient care outcomes.
Demonstrates professionalism, integrity, and accountability in the LPN role.
Complies with LifeBridge patient safety mission, policies, and procedures.
Certifications & Licenses
American Heart Association CPR Certification
License for Practical Nursing
Wound Care Certificate within 2 years of hire
Ostomy & Hyperbaric Certificate (when applicable, WC)
40‑hour UHMS HBOT course within 1 year of hire (WC)
Education / Knowledge Formal working knowledge; equivalent to an Associate’s degree (2 years of college); requires knowledge of a specialized field.
