Responsibilities:
Acts as client advocate in order to coordinate required services or to resolve emergency problems in crisis situations.
Collects information about clients through interviews and observations.
Utilizes skills trainings with clients in both individual and group sessions in order to help them overcome deficiencies, recover from illness, and better adjust to life.
Develop and implement treatment plans based on clinical experience and knowledge.
Insures confidentiality of records relating to client’s treatment.
Evaluates client’s physical or mental condition based on a review of information.
Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plans.
Insures safety and security of clients and co-workers. Must have good knowledge of agency safety policies and procedures.
Completes doc via EMR in a timely manner.
Other duties as assigned.
Qualifications Needed (education and/or experience):
Bachelor’s degree in related field and two years, Preferred of related experience or a Master’s degree in a related field. Or one year at a lower level or equivalent position.
Completes/maintains continuing education requirements. Examples include: Mindset CPR/FA/AED, Essential Learning, and others as required.
Good interpersonal skills. Has the ability to observe, record, and report deviations in patient’s conditions.
Good communications skills, both from an interpersonal standpoint and the ability to write/produce written reports/documentations as needed to insure good patient care.
Physical Requirements:
Must be able to lift 16 – 20 pounds whenever needed.
