Nurse Practitioner Job Description This is a full-time position, working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of skilled nursing facility residents. The APRN will establish and build strong working relationships with administrators and staff throughout their designated building(s) and will act as the primary point of contact for emergencies and urgent care visits. Ultra Health Providers provides our APRNs with the training and tools to do more of what they love – working independently to see patients and treat complex medical conditions.

What Does a Day Look Like for a UHP APRN?

Provide primary, urgent and preventative care for residents in assigned skilled nursing facility, may also provide care to assisted living facility residents.

Verify admission orders and conduct initial assessment of residents, conduct physical exams, medication review, and depression screening.

Collaborate with facility Medical Director, attending physicians and nursing staff to deliver the highest level of care to the patient.

Communicate with and counsel family members on treatment options when there are material changes in status.

Nurse Practitioner Minimum Qualifications

Valid Nurse Practitioner License in Georgia

DEA License

Nurse Practitioner national certification

A clinical background in adult, family or geriatric specialties

Vaccination against COVID-19 is required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.

Nurse Practitioner Preferred Qualifications

Experience in internal medicine, gerontology, or cardiology

Excellent administrative, organizational and verbal skills

Ability to work independently

Detail oriented

Dependable and reliable

Nursing experience in LTC / SNF a plus

Compensation $100,000 - $150,000 a year
