Responsibilities
Lead and coordinate Minimum Data Set (MDS) assessments, care planning, and RAI processes. Collaborate with interdisciplinary teams to support high‑quality resident care. Ensure compliance with CMS guidelines and regulatory requirements. Contribute to raising the quality of care and fostering a resident‑centered environment. Provide oversight of weekly PDPM meetings and monitor effective utilization of PDPM tools. Track and monitor QM and QASP performance monthly and quarterly; provide data analysis and clinical recommendations. Provide PDPM and MDS training to new MDS Coordinators. Assess and determine health status and level of care for new admissions. Ensure accurate and timely completion of all MDS assessments. Communicate level of care for new residents to all disciplines. Coordinate interdisciplinary participation in completing MDS for each admission and resident. Maintain accurate schedule of MDS assessments and reference dates. Perform data entry, verify electronic submissions, and correct records as necessary. Schedule and conduct resident care conferences in compliance with regulations. Assist disciplines in formulating and revising care plans. Ensure reports of resident problems, goals, and nursing interventions are precise. Evaluate care plans and revise them appropriately. Manage all level‑of‑care changes and notify departments. Generate and transmit required forms for acuity changes. Maintain confidentiality of resident care information and
