Deliver high-quality, evidence-based physical therapy treatments in patients' homes Implement individualized treatment plans developed by supervising physical therapists Promote safety and functional independence in the home setting Educate patients and caregivers on exercises, mobility, and home safety Document patient progress accurately in compliance with Vanderbilt Health and regulatory standards Coordinate care with nurses, physicians, and allied health professionals Travel to patient homes across designated service areas Required Qualifications

Active

Physical Therapist Assistant (PTA)

license in Tennessee Minimum one year of experience as a PTA Excellent communication and interpersonal skills Ability to work independently and manage a flexible caseload Valid driver's license and reliable transportation Preferred

Experience with EMR systems Strong understanding of rehabilitation techniques and functional mobility Ready to make a real impact?
