Overview
RN-Home Health – AMEDISYS HOME HEALTH Shift: Fulltime M-F (Call every 3rd weekend) Tired of being stuck in a building for 12 to 13 hour shifts all day? Responsibilities
Demonstrates knowledge of home care and competency in discipline-specific patient care skills required by the care center for the provision of patient care. May be assigned RN visits as a member of the care team, and/or may be required to supervise a patient caseload, including LPNs and HHAs. Performs patient comprehensive assessments, collaborates with the patient’s multidisciplinary care team (PT, OT, ST, MSW and paraprofessionals) and other health care professionals to develop/modify and implement an individualized plan of care per physician orders that ensures quality, proper discharge planning and achievable outcomes. Facilitates timely and effective communication of care plan changes, new orders, lab results, and other information among all team members to maintain continuity of patient care. Provides patient episode management per company protocol. Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so. Utilizes standard practices to achieve positive clinical outcomes and participates in clinical outcomes monitoring and care center improvement initiatives. Initiates, plans and provides staff education and performs competency validation for staff as needed. Uses approved patient/family education materials and regularly communicates patient progress to the clinical manager. Completes documentation timely and accurately at the point of care, including OASIS assessments, SOC/Admissions, Recertifications, ABNs, visits, and care coordination. Performs other duties as assigned.
Qualifications
Current, unencumbered license to practice as a Registered Nurse, valid in the state(s) of assignment. May possess a single-state license or a multi-state (Nurse Licensure Compact) license as applicable. Nurse Licensure Compact (NLC) documentation acceptable in participating states. One (1) year or more of clinical RN experience; if less than 1 year, candidate must be approved by VP Clinical. Current CPR certification. Ability to assess patient status and address age-specific needs. Proficiency with computer use (basic functions, e-mail, online HR systems) and ability to manage online training. If less than 6 months of clinical RN experience, candidate must participate in RN Intern program.
