Practical Nurse (PACT) – U.S. Department of Veterans Affairs Position: Practical Nurse (PACT) at Dayton VA Medical Center. Pay range: $45,601.00 – $81,022.00 per year. Grade: GS‑03 to GS‑06.

Summary

The Dayton VA Medical Center is seeking a Practical Nurse in the Primary Care Service. The LPN will perform duties that promote quality health care, provide positive customer service, and maintain an environment committed to serving veterans.

Qualifications

Applicants pending completion of educational or certification/licensure requirements may be referred and tentatively selected but must not be hired until all requirements are met.

Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a U.S. state, territory, Commonwealth, or District of Columbia.

English language proficiency in written and spoken English.

United States citizenship (non‑citizens may be appointed under specific circumstances).

Military service health‑care education or training accepted by the licensing body qualifies as meeting the education requirements.

Basic Requirements by Grade

GS‑03: None beyond basic requirements.

GS‑04: Six months of qualifying experience as an LPN or LVN, or graduation from an approved school and one year of experience in nursing care.

GS‑05: Completion of at least one year of qualifying experience at the GS‑4 level or equivalent.

GS‑06: Completion of at least one year of additional qualifying experience at the GS‑5 level or equivalent.

Physical Requirements

Visual acuity, keen hearing, clear speech, and manual dexterity.

Ability to walk, stand, stoop, sit, bend, pull, and push for prolonged periods.

Capability to transfer patients and objects.

Exposure to infected patients, contaminated materials, protective clothing usage, and occasional combative patients.

Adaptability to stressful situations and shift priorities based on patient needs.

Duties

Notify supervisory staff of any changes in patient condition and significant patient events.

Complete clinical reminders, documentation, and assist in completing patient forms.

Provide direct patient care for male and female patients across all age groups, including the elderly.

Demonstrate competency in population‑based nursing care and knowledge of physiological and psychosocial changes related to aging.

Deliver nursing care according to principles of Patient‑Centered Care, PACT, and related standards.

Recognize and respond to critical or emergency patient situations, maintaining BLS certification and performing CPR when indicated.

Demonstrate safety skills in emergency preparedness, hazardous materials, fire plans, equipment failure, and utility outage procedures.

Participate in group clinics (hypertension, hyperlipidemia, diabetes, etc.).

Record nursing care observations, procedures, vital signs, treatments, medications, and update personalized health plans.

Maintain a clean and safe environment; assist in routine unit activities; perform other duties as assigned.

Collaborate with interdisciplinary team on patient treatment plans; practice universal precautions.

Model professional conduct and self‑control in stressful situations; treat all patients, families, and coworkers with respect.

Provide care that meets or exceeds veteran expectations; address customer‑service concerns by referring to supervisor or charge nurse.

Assist in benefits-related responsibilities, including work schedule and paid time off details as noted.
