Responsibilities
Represent the branch in external transitional care activities and maintain professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and other health groups.
Develop patient‑centered transition plans by integrating clinical guidelines, protocols, and other metrics to promote quality and efficiency.
Act as a public awareness representative and educate stakeholders about Enhabit services.
Qualifications
Graduate of an approved nursing school.
Licensed in the state of practice.
At least one year of experience in home care.
Demonstrated experience with federal, state, and local hospice regulations.
Basic technology skills, including mobile device operation.
Preferred
Experience and knowledge in hospice care.
Requirements
Valid state driver’s license.
Maintain automobile liability insurance as required by law.
Responsible, dependable transportation in good working condition.
Able to drive safely in all weather conditions.
