Location: Carilion Franklin Memorial Hospital (CFMH), Rocky Mount, VA. Shift: Day (8:00am–5:30pm). Full‑time (4–9 hour days).
Responsibilities
Assess, plan, implement, and evaluate patient care to achieve optimum outcomes for patients of all ages, as appropriate to the Cardiac Rehab service.
Complete age‑specific admission and ongoing assessments, document, and develop a care plan.
Collaborate with the interdisciplinary team, patients, and families to develop, implement, and evaluate an age‑specific plan of care, revising it as patient status changes.
Accurately administer and document treatments and medications using the automated computer system, including EPIC.
Identify and document patient/family educational needs, incorporate into the plan of care, and implement them appropriately.
Contribute to Carilion Clinic’s performance‑improvement activities.
Serve as patient advocate, protector of rights, and provider of psychological/social/cultural support.
Apply knowledge of growth and development to appropriately care for patients served.
Support evidence‑based clinical and quality improvement initiatives and leadership pathways.
Exhibit clinical expertise in rapidly changing circumstances and plan strategies to reduce length of stay and improve outcomes.
Act as role model for clinical practice; manage resources and prioritize patient care needs.
Complete manager/director projects within specified timelines and guidelines.
Utilize problem‑solving methods to resolve patient and personnel issues.
Actively orient new employees and provide timely feedback to staff.
Participate in unit, hospital, and system activities to facilitate staff input into decision‑making.
Assist with education activities for the unit or department.
Qualifications
Graduate of a professional nursing school or an approved program eligible to sit for the VA licensure exam.
Currently licensed as a Registered Nurse in Virginia OR eligible to obtain Virginia RN licensure.
AHA BLS‑HCP required; may also need BLS, ACLS, PALS or NRP based on unit requirements.
Computer skills for EPIC electronic medical record.
