Title: Supports Coordinator, Exempt Staff Definition: Under the direction of the Director of Supports Coordination Services Location: Montgomery County caseloads – travel required to participants’ homes and community locations. Salary Range: $45,000 to $55,000 per year. Benefits: Employer insurance contribution for employee, spouse and dependents. Brief Description
Supports Coordinators provide case management services to individuals of all ages with intellectual and developmental disabilities living in various community living arrangement programs or their own/family homes throughout Berks, Bucks, Chester, Delaware, Montgomery and Philadelphia counties. The Coordinator monitors health and safety, locates and coordinates services and supports—including Medicaid waiver program or community resources—and maintains Individual Support Plans (ISPs). Responsibilities
Coordinate and provide comprehensive case management services for eligible individuals. Monitor health and safety of clients and deliver follow‑up support as needed. Locate and coordinate services and supports through Medicaid waiver programs and community resources. Develop, implement, and maintain Individual Support Plans (ISPs). Record progress, modify plans, and ensure services are delivered in a timely manner. Conduct individual and family interviews, assessments, and plan reviews. Assist clients and families in accessing available resources and services. Respond to emergencies and facilitate crisis interventions. Participate in community activities and agency fundraising efforts. Requirements
Experience and understanding of local, state, and federal special education systems; Pennsylvania MH/ID service systems; welfare, social security, and other benefit systems. Working knowledge of agency and community services and their relationships to programs and families. High‑quality organizational and time‑management skills; ability to work in a high‑pressure environment. Excellent communication and writing skills for interfacing with county personnel, program coordinators, contract coordinators, staff, therapists, and other agencies. Empathy and ability to work effectively with individuals who are physically challenged, emotionally troubled, or economically disadvantaged. Planning, organizing, record‑keeping, and caseload management skills. Qualifications
Legal authorization to drive in Pennsylvania; valid driver’s license; current automobile insurance. 3‑year driver record check at hire, updated annually; recommended coverage limits of $100,000 liability / $300,000 third‑party / $100,000 property damage. Current FBI, Criminal History, and Child Abuse clearances required at hire; maintain clearances as per regulations. Physical ability to drive throughout the counties served. Physical Requirements
Frequent verbal and written communication; frequent sitting and standing; visual tasks and other duties as required. Seniority Level
