Position Overview We’re seeking a compassionate and dedicated Physical Therapist to join a collaborative care team. This role is perfect for professionals passionate about improving patients’ mobility, functionality, and overall well‑being.
Key Responsibilities
Conduct comprehensive evaluations and develop individualized treatment plans
Deliver evidence‑based therapeutic interventions
Coordinate with other healthcare providers for integrated patient care
Educate patients and families on exercises and long‑term wellness strategies
Maintain accurate and timely clinical documentation
Must‑Have Qualifications
Doctor of Physical Therapy (DPT) or Master’s in PT (grandfathered license holders welcome)
Active or eligible Wyoming PT license
Excellent interpersonal and communication skills
Ability to work both independently and as part of a team
Nice‑to‑Have Qualifications
Experience or certification in Lymphedema or Pelvic Health
Prior home health or outpatient care experience
Familiarity with WebPT (preferred EMR)
