Responsibilities Evaluate patients and develop individualized treatment plans Provide physical or occupational therapy services in accordance with clinical protocols Monitor patient progress and adjust treatment plans as needed Document evaluations, treatments, and outcomes in the EMR Collaborate with interdisciplinary teams to support patient care goals Educate patients and caregivers on treatment plans and functional strategies
Requirements Active New York State license as a Physical Therapist or Occupational Therapist Experience in hospital, outpatient, skilled nursing, or long-term care settings preferred Strong clinical judgment and documentation skills Patient-centered, collaborative approach to care New graduates are welcome to apply
