Nurse Practitioner, Paneled – CenterWell Senior Primary Care (Senior Primary Care)
Overview The Nurse Practitioner, Paneled applies advanced education and clinical competencies to achieve optimal patient panel outcomes. This role involves diagnosing, treating acute and chronic conditions, prescribing medication, and advising on functional strategies. The NP works independently on complex issues with minimal supervision and serves as a primary care provider for the assigned patient panel.
Responsibilities
Provide health promotion and maintenance through diagnosis and treatment of acute illness and chronic conditions.
Prescribe medication, examine patients, diagnose illnesses, and provide appropriate treatment.
Advise executives on functional strategies related to patient care and organizational performance.
Exercise independent judgment and decision-making for complex issues, ensuring adherence to state scope of practice and collaboration requirements.
Coordinate care with the patient care team.
Participate in limited on-call coverage by phone.
Qualifications
Bachelor's degree.
Minimum 8 years of technical experience.
Minimum 2 years of project leadership experience.
Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
Active, unrestricted DEA license.
Appropriate certification and credentials to write prescriptions per state and federal regulations governing nurse practitioners.
Passion for contributing to an organization focused on continuously improving consumer experiences.
Willingness to participate in limited on-call coverage by phone.
Schedule 40 hours per week.
Pay Range Base pay: $126,300 – $173,700 per year. Possible additional bonus incentive plan based on company and individual performance.
