Care Coordinator – Vinton County, OH Integrated Services for Behavioral Health (ISBH) is a community-minded, forward‑thinking behavioral health organization helping people along the road to health and well‑being. We meet people in their homes and communities and help connect them to their needed resources. We serve southeastern and central Ohio with a comprehensive array of behavioral health and other services – working with local partners to promote healthy people and strong communities.

Pay range:

$20.19 - $25.03 per hour based on experience, education, and/or licensure.

Responsibilities

Join with families to identify care coordination needs and services in line with program outcomes.

Work with families to define cultural factors influencing strengths, functioning, and interaction styles.

Identify strengths of children, youth, and families for utilization in care coordination engagement.

Coordinate family‑based services for children, youth, and families in their home, school, and community.

Ensure services on care plans are the most appropriate, least restrictive, and meet safety and treatment needs.

Engage and build positive relationships with children, youth, and families in coordination with child and family teams.

Develop collaborative partnerships with community resources to meet diverse needs.

Maintain documentation, participate in program evaluation, attend team meetings, cross‑systems training, and acquire knowledge of community resources.

Remain current with all training requirements, including High Fidelity Wraparound, MI, Cultural Humility, etc.

Perform any other duties as assigned.

Minimum Requirements

Three years of experience with a high school diploma or equivalent; or

Two years of experience with an associate degree or bachelor's degree; or

One year of experience with a master’s degree or higher.

Knowledge and experience in Hi‑Fidelity Wraparound preferred (certification provided at employment).

Two years of experience in a coordinated supportive services or care coordination role preferred.

Experience working with individuals with autism spectrum disorders and developmental disabilities preferred.

Additional Qualifications

Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, child development.

Be culturally humble or responsive with training and experience to manage complex cases.

Qualifications to work with children and families experiencing serious emotional disturbance, trauma, co‑occurring behavioral health disorders, with engagement in child‑serving systems.

Excellent organizational skills with ability to prioritize multiple tasks.

Demonstrated high cultural awareness.

Experience with multi‑need individuals and families.

Broad knowledge of community service systems.

Willingness to participate in and lead cross‑systems care coordination.

Able to communicate effectively through verbal and written expression.

Proficient in an internet‑based, automated office environment.

Valid driver’s license required.
