Overview
Clinical Intake Specialist (Nurse, Office, LPN) - HomeCare at Hartford HealthCare. Location Detail: 1 Northwestern Dr Bloomfield (10320). The Region Clinical Intake Specialist is responsible for managing and coordinating the clinical intake process for Home Health and Hospice services across multiple branches. This role ensures timely, accurate, and compliant intake procedures using the Homecare Homebase (HCHB) system, serving as a liaison between referral sources, clinical teams, and operational leadership. Responsibilities
Perform all clinical aspects of the intake process, including communication with patients and families and admitting clinicians, ensuring clinical data entry accuracy, timeliness, and Face to Face encounter accuracy and PCP/MD verification. Ensure patients are admitted timely and in accordance with intake policy. Provide recommendations for process and policy improvements and ensure compliance with all state, federal, and Joint Commission referral/intake regulatory requirements. Stay informed about clinical quality metrics and maintain knowledge through engagement with Home Care Leadership, healthcare networks, or other partners for Home Health and Hospice services. Ensure patient admissions comply with contract rules, requirements, and terms. Establish and maintain positive working relationships with referral sources and associated clinical leadership teams. Coordinate care for patients to home care by overseeing patient education, care planning, and coordination with multiple providers, case managers, and other clinical needs within established payer and financial guidelines. Strengthen and monitor community and customer perceptions of Hartford HealthCare at Home as a high-quality service provider, and collect/report clinical referral data and key customer trends. Perform other job-related tasks as assigned. Qualifications
Education: Licensed clinician (RN, LPN, or equivalent) required. Bachelor’s Degree Preferred (BSN, Healthcare Administration or related field). Experience: 1 year of Administrative/Clinical Healthcare Experience; 1-3 years of experience preferably in Home Care. Knowledge, Skills and Abilities: Healthcare experience in referrals/intake in a home health or hospice environment; strong communication, negotiation, and public relations skills; autonomy, assertiveness, flexibility, and teamwork in performing responsibilities.
