Experienced LCSW passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery.
Provide therapeutic interventions and resource recommendations that address patient’s goals to achieve personal, career, and independent living goals in an integrated setting.
Value professional autonomy, teamwork, communication, and collaboration to maximize patient recovery.

Responsibilities
Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
Set measurable objectives that are formulated in conjunction with the rehab team.
Assist the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems.
Instruct or counsel patients and families in treating and coping with social and emotional responses connected with illnesses.
Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs.
Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives.
Qualifications
Master’s Degree in Clinical Social Work (LCSW).
Current Licensed Clinical Social Worker licensure in the state of practice.
Minimum one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis.
Current BLS (Basic Life Support), First Aid and CPR certification required.
Ability to lift 50 pounds; lifting or transferring patients up to 100 pounds following safety procedures.
Fine motor skills, visual acuity, and walking/standing for extended periods.
Physical tasks include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
Salary Range USD $50.00 - $55.00 / Hour
