Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants to join the integrated Care Team composed of a Registered Nurse and a Social Worker. The team focuses on patients with chronic conditions, especially CKD and/or ESRD/ESKD, aiming to improve quality of life and slow disease progression.
Job Description The Advanced Practitioner is a key member of an integrated Care Team. Their role includes delivering compassionate care to patients mainly with CKD, administering follow‑up appointments, maintaining accurate records, and coordinating care with physicians and ancillary services.

Roles and Responsibilities
Conduct assessments, including comprehensive annual wellness exams in patients’ homes and virtually
Counsel and educate patients and families about benefits and programs to promote healthy living
Document clinical information: chief complaint, diagnosis, history, review of systems, exam, meds, allergies, assessment, plan
Coordinate care with primary care providers, specialists, and ancillary services
Complete documentation and paperwork in a timely manner
Maintain quality of care standards set by the practice
Work remotely in state of employment, attending periodic training/meetings outside the state
Deliver evidence‑based care that reduces avoidable hospitalizations and maximizes quality of life
Prescribe medications, order tests, collaborate with Monogram physicians
Achieve performance metrics reflected in daily patient outcome reports
Education patients on preventive monitoring and scheduling diagnostic testing
Assist patients in enrolling for educational videos
Participate in integrated care team meetings
Apply knowledge of disease diagnosis and prevention
Make health assessments, develop treatment plans, implement plans, and follow up on patient status
Perform other duties as assigned
Position Requirements
Bilingual (English/Spanish) highly preferred
Active, unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
Board certified (NP: ANCC/AANP; PA: NCCPA)
Current, unrestricted DEA certificate
Ability to work independently and autonomously
Valid driver's license, car insurance, and reliable transportation
Proficiency with medical instruments and equipment required for the role
Knowledge of computer‑based data management, electronic medical records, and point‑of‑interview technology
Excellent verbal and written communication skills with patients, families, physicians, and community partners
Understanding of federal and state regulations, including HIPAA and OSHA
2+ years of direct patient care experience
Managed Care/IPA/Health Plan experience
Experience conducting annual wellness visits or similar visits virtually or in the home
