Role Description
This is a consistent PRN, on‑site role for a Physical Therapist, based in Cary and/or Durham. The therapist creates and sets their own patient schedules. The Physical Therapist will provide in‑home rehabilitation services, focusing on patient care and improvement of functional mobility. Responsibilities include developing individualized therapy plans, implementing treatment strategies, and collaborating with patients, caregivers, and other healthcare professionals to optimize therapeutic outcomes. Documentation of patient progress and maintaining compliance with therapy standards are essential components of the role.
Qualifications
Expertise in Physical Therapy, Rehabilitation, and Patient Care
Knowledge and experience in Medicine, with a focus on geriatric care
Demonstrated proficiency in delivering Home Care services
Strong communication, assessment, and interpersonal skills
Ability to develop and implement individualized care plans
Valid state licensure to practice as a Physical Therapist and a degree in Physical Therapy or a related field
Experience in dementia care and geriatrics is highly preferred
