Pediatric Home Team (Transitional) Nurse

Use strong clinical and assessment skills to assess patients’ post‑acute needs in collaboration with the Halos Home Team and patients’ discharge care providers to ensure continuity of care when discharging home. Essential Duties and Responsibilities
Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their homes according to the active plan of care. Conduct ongoing patient care and assessments. Administer prescribed medications, treatments, and therapies. Coordinate care. Educate family members/caregivers on patient clinical care to enhance positive outcomes. Preventative initiatives to protect quality of care. Assess patients’ post‑acute care needs with Clinical Care Managers and discharge providers to ensure continuity of care. Provide interim care during gaps due to call‑offs, vacations, and staff shortages. Provide one‑on‑one training to nurses in the field to ensure smooth clinical and non‑clinical transition. Serve as a liaison and patient advocate on complicated cases. Performance Responsibilities
Exercise cost‑control measures. Maintain positive internal and external customer service relationships. Demonstrate effective communication skills and active listening. Plan and organize work effectively and ensure its completion. Meet all productivity requirements. Promote a team‑oriented environment. Represent the organization professionally at all times. Demonstrate initiative and continuously improve processes and relationships. Qualifications / Requirements
Active Texas RN or LPN license. Active CPR certification. Special Requirements
Flexibility in scheduling with the ability to work on call as needed. Physical Demands & Working Conditions
Ability to lift up to 50 pounds. Work under stressful conditions to meet deadlines and patient needs. Ability to make quick decisions and allocate resources. Work in routine office environments, local travel, and occasional inclement weather. Work in patient homes, possible exposure to blood and bodily fluids, and flexible schedule. Benefits
