Behavioral Health Care Coordinator
Behavioral Health Solutions (BHS) is a team of mental health treatment professionals that specializes in providing comprehensive behavioral health services for adults and geriatrics in a variety of settings. BHS encourages a collaborative culture with hands‑on leadership. We are seeking skilled professionals dedicated to delivering state‑of‑the‑art psychiatric services.
As a Behavioral Health Care Coordinator, you will be an integral part of our team, coordinating and providing essential services to patients in long‑term care and skilled nursing facilities. Your role is to cultivate positive relationships with each facility in your assigned region, ensuring seamless coordination of services among BHS, facility staff, providers, and patients.

In‑office work with travel to SNF and LTC facilities in the community
Responsibilities
Travel to all locations within your assigned region to provide onsite support and services as required.
Engage and collaborate with facility staff, including Executive Directors, Directors of Nursing, case managers, social workers, staff developers, admissions personnel, surveyors, and physicians.
Coordinate a variety of services—psychiatry, therapy, and neuropsychology (where available)—to meet patients’ needs.
Assist facilities in processing referrals and identifying residents who could benefit from psychiatric and therapy services.
Coordinate completion of consents and provide assistance when needed.
Educate facility staff on BHS processes, address questions, and resolve concerns.
Attend behavior‑management meetings with facility staff and coordinate MD attendance for facility meetings such as GDR and QAPI.
Ensure compliance with PASRR II requirements in partnership with BHS and the facility.
Schedule in‑services and educational sessions for facility staff.
Facilitate telehealth visits with clinicians as needed to enhance patient care.
Support the facility onboarding process in collaboration with the Business Development team.
Perform additional tasks and assignments as specified by your supervisor.
Competency
Knowledge of the survey process in skilled nursing facilities.
Behavioral/medical terminology, therapy, pharmacology, and related procedures.
Strong organizational skills, attention to detail, data‑driven mindset, and proficiency in technology.
Adaptability to multitask in a fast‑paced environment with evolving opportunities.
Effective critical‑thinking and problem‑solving skills for patient care coordination.
Ability to build and maintain relationships with facilities, providers, and supervisory teams.
Excellent written and verbal communication skills.
Current COVID‑19 vaccination to comply with our safety protocols.
Requirements
Ability to perform each essential duty satisfactorily.
High school diploma or GED; some college education in behavioral health or psychology preferred.
3‑5 years’ experience in skilled nursing facilities as a licensed social worker, support staff, or clinical care coordinator.
Proficiency with a PC in a Windows environment, internet usage, and MS Office (Outlook, Word, Excel); proficiency in an EHR system (Athena) preferred.
Working Conditions
Remain stationary more than 50% of the time during administrative tasks.
Walk and move frequently throughout the office and skilled nursing facility to access files, machinery, contacts, and patients.
