Position Summary This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. It assists with informed decision-making, collaborates with a multidisciplinary team to enable timely screening, diagnosis, treatment, and increased supportive care throughout the patient experience.

Core Functions

Assesses the patient’s plan of care and develops, implements, monitors, and documents the utilization of resources and progress of the patient.

Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes.

Assists the patient/family through diagnostic services, treatment and care, coordinating disease care using an interdisciplinary holistic approach.

Ensures that the patient and family understand diagnostic processes, care strategy and recommended actions; provides appropriate information and responds to patient requests.

Evaluates patient’s functional abilities, determines intervention needs, establishes and implements a plan of care to achieve treatment goals.

Supports the patient and family during decision-making periods, including end-of-life care coordination and emotional support.

Supports process improvement activities for patient populations, participates in staff development, and maintains current standards of practice.

Establishes collaborative relationships with physicians, payers, and other team members, collecting and communicating timely information to fulfill utilization and regulatory requirements.

Works independently within established procedures, consulting supervisor on unusual situations; determines best ways to accomplish functions.

Minimum Qualifications

Knowledge obtained through completion of a Bachelor’s degree in nursing or related field.

Current RN license in the state of practice and BLS certification.

Approximately five years of clinical experience with excellent organizational and communication skills.

Knowledge of specialty care services, care coordination, legal and financial aspects of diagnostic services, and health services in specialty areas.

Proficiency with word processing and database software applications.

Ability to teach clinical and non-clinical personnel regarding care and diagnostics services, with a good understanding of process improvement.

Preferred Qualifications

Current certification in specialty area.

Prior case management experience.

Additional Related Education And/or Experience Preferred Current certification in specialty area or prior case management experience is preferred.
