Hospice Medical Social Worker (MSW) – Affinity Health Management

Job Overview The MSW provides medical social work services to hospice patients and families. They are core members of an Interdisciplinary Group and offer psychosocial support based on ongoing assessment of needs and identified goals. Essential Job Functions / Responsibilities

Assesses psychosocial status of patients and families related to terminal illness and communicates findings to the interdisciplinary group. Conducts social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risk for suicide, neglect or abuse, and implements intervention based on findings. Provides counseling to patients and families and offers crisis intervention when necessary. Educates the interdisciplinary group on cultural needs of patients and families. Prepares advanced directives and assists with funeral, financial, legal, and healthcare decision responsibilities. Participates in individualized plan of care, attends interdisciplinary meetings, and supports the team in recognizing psychosocial stresses. Communication

Maintains accurate clinical notes regarding patient condition and care. Communicates with physicians regarding patient needs and reports changes; obtains physician orders as required. Coordinated with community health related persons to align the care plan. Provides information and referral services to patients, families, and community agencies. Serves as liaison between patients/families and community agencies. Reports mandated abuse/neglect and suspects to appropriate authorities. Ensures a safe environment for patients and continuity of care. Additional Duties

Participates in on-call duties, as defined by policy. Supports RN Case Manager with arrangement of equipment and necessary services. Maintains professional knowledge through continuing education. Participates in quality assessment and performance improvement teams. Fulfills other delegated duties as assigned. Position Qualifications

Graduate of a Master’s program in Social Work (MSW) from a school accredited by CSWE. Minimum of 1 year experience in health care; hospice experience preferred. Strong verbal and written communication skills; organized. Licensed driver with a valid insurance; good driving record. CPR certification. Valid references and criminal background checks required.
