Job Summary The Patient Care Coordinator – Registered Nurse (PCC‑RN) functions as an integral member of a team of care providers on a patient care unit, managing care for patients and their families while directing and supervising other members of the patient care team. The PCC‑RN assesses, prescribes, implements, coordinates, delegates, and evaluates nursing care of patients, prioritizes patient care needs, and provides leadership and development for the patient care team.
Responsibilities
Manage care to patients and families, ensuring comprehensive, patient‑centered service.
Assess, prescribe, implement, coordinate, delegate, and evaluate nursing care for patients in a medical‑surgical setting.
Provide leadership and developmental support to the unit team (LPN, PCA, UCA).
Work collaboratively with other providers to achieve patient‑care outcomes, maximize patient involvement in their care plan, and reduce length of hospitalization and costs.
Qualifications
Graduate of an accredited school of registered nursing.
Current Ohio RN licensure or interim permit.
Previous nursing experience in a relevant care setting is preferred.
Maintains up‑to‑date knowledge of patient‑care issues, quality improvement, cost reduction, and current policies, procedures, and protocols.
Meets mandatory educational and health requirements.
Additional Information
Demonstrates computer skills required to meet job functions.
Final candidates are subject to successful completion of a background check; a drug screen or physical may be required during the post‑offer process.
