Note: Min of 3 years of clinical or home health experience required. Must be licensed and reside in Texas.
What you’ll do:
Make outbound calls to patients to close care gaps and support preventive care
Assist with scheduling screenings, follow-ups, and appointments
Coordinate with primary care providers and healthcare teams
Review reports to identify quality gaps and support value-based care goals
Educate patients and families on health management and medication adherence
Support high‑risk patients with multiple health conditions
Focus Improving patient outcomes, quality performance, and continuity of care.
