Transfer Center Registered Nurse / RN – Emory Healthcare Emory Healthcare is seeking a Transfer Center Registered Nurse to accept incoming referral information via multimedia and coordinate inpatient bed assignments, especially for critically ill patients. The role works directly with bed management and direct care nursing to optimize patient flow across the hospital network.

Responsibilities

Coordinate, prioritize, and optimize patient flow using clinical and operational data.

Collect clinical information to determine patient’s level of care and collaborate with providers for acceptance by an attending EHC physician at the most appropriate EHC hospital.

Monitor bed availability and coordinate bed assignments for all patients receiving care in inpatient or outpatient beds and for transfers between units and from outside facilities to maximize bed utilization.

Communicate with referring hospitals to coordinate transportation arrangements and provide an assigned bed.

Communicate clinical information to transportation companies, physicians, physician designees, referring facilities, and charge nurses.

Ensure safe and timely placement of patients based on admission status, physician requested level of care, patient clinical information, and nursing unit admission/discharge criteria.

Maintain the patient transfer and flow software to accurately reflect physical location of all patients occupying an inpatient bed and/or ancillary holding area.

Confirm patient insurance eligibility and obtain specific payor benefits.

Obtain and confirm admission notifications and authorizations for external clinic/facility transfers.

Document findings in the Financial Billing System and exhibit appropriate customer/guest relations in all interactions.

Consistently communicate information in a timely, accurate and concise manner.

Apprise Manager of significant developments affecting patient flow.

Participate on Nursing and Hospital committees and councils as required.

Perform other duties as assigned.

Minimum Qualifications

Bachelor’s degree in nursing required.

Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.

Three years of recent intensive care nursing experience within the past five years that includes supervision and patient bed placement.

Strong critical thinking, accurate assessment, problem solving, excellent verbal and written communication skills, attentiveness to detail, high-level analytical ability, strong interpersonal skills, knowledge in nursing concepts, practices and procedures.
