Advanced Practice Provider (FT) - Community-Based Family Medicine
The Dartmouth Health's Department of Community and Family Medicine is seeking a Family Medicine APRN or PA to join our academic Primary Care practice at the Heater Road Clinic in Lebanon, NH. The ideal candidate will have at least 5 years of direct primary care provider experience, be an enthusiastic teacher and scholar, and be committed to improving patient health through high‑quality team‑based care in an academic setting. The new team member will provide healthcare to individuals and families, assess and manage health care needs and issues within the specialty area, emphasizing health promotion, disease prevention, diagnosis and treatment of acute and chronic illnesses, and may assume primary responsibility for patient care.
The Heater Road Clinic is located in a beautiful outpatient facility three miles from Dartmouth‑Hitchcock Medical Center.
We seek candidates with 3+ years of primary‑care experience, ideally in a community setting.
The Family Medicine team at Heater Road is part of a dynamic Primary Care group focused on clinical and educational quality improvement, population health, and new models of care delivery.
Our training includes specialties such as Sports Medicine, Sleep Medicine, Functional Medicine, and Addiction Medicine, and we offer leadership development opportunities.

Responsibilities Provide primary care to individuals and families, assess health conditions, develop and manage treatment plans, perform acute and chronic disease management, collaborate with the provider team to deliver high‑quality patient care in community‑based settings, and assume primary responsibility for patient care.
Qualifications
Graduate of an accredited APRN or PA academic program
National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA)
Required Licensure/Certifications
Eligible for RN licensure in the State of NH (APRN only)
Eligible for APRN or PA licensure in the State of NH
Eligible for Federal DEA Registration
BLS certification
