Nurse Practitioner – Per Diem

Position Summary : The Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/or palliative care to patients and families. The NP works cooperatively with members of the interdisciplinary Hospice team, acts as a consultant to the team, and participates in IDG meetings. The NP regularly reviews and updates the plan of care, performs prescribed medical treatments, including patient examination, diagnosis, medication prescription, pain management, and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients, and caregivers. The NP examines and diagnoses patients and prescribes medications.
Responsibilities :
Deliver hospice and/or palliative care in collaboration with the Medical Director/Physician.
Consult with and collaborate with interdisciplinary team members; participate in IDG meetings.
Review and update patient care plans regularly.
Perform patient examinations, diagnoses, prescription of medications, pain management, and symptom control.
Conduct assessments and evaluations, and educate hospice teams, patients, and caregivers.
Prescribe medications and treat patients as authorized nurse practitioner.
Cover the San Fernando Valley areas.
Qualifications :
Master's or Doctoral degree in nursing.
Current NP License within the State; certification in Hospice and Palliative Nursing desired.
Minimum of two-year experience as an NP within the last 3 years.
Experience in Palliative/Hospice Care highly preferred.
Compensation : $150 per visit + Administrative Compensation.
Seniority level : Mid-Senior level
