LPN Licensed Practical Nurse

Base Pay Range $35.00/hr – $45.00/hr

Job Description Department: Nursing Services
Reports to: Director of Nursing, Assistant Director of Nursing, Unit Nurse Manager
Supervises: Certified Nursing Assistants, Medication Technician/Aide, Restorative Aide
Position Purpose Provides direct nursing care to residents and supervises day-to-day nursing activities performed by certified nursing assistants, ensuring compliance with federal, state and local regulations and facility policies.
Required Qualifications
Nursing Degree from an accredited college or university or graduation from an approved LPN/LVN program.
Prior experience in long-term care preferred but not required.
Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the practicing state.
Current CPR certification.
Major Duties And Responsibilities
Direct daily activities of certified nursing assistants according to regulations and facility policies.
Ensure compliance with all policies and procedures.
Participate in admission, discharge, and transfers of residents.
Observe changes in residents’ status, notify physician and family, and document accordingly.
Transcribe physician orders to medical record and carry out orders as written.
Prepare and administer medications per physicians’ orders and monitor for adverse effects.
Perform wound treatments as ordered, observe changes, and document.
Schedule follow-up appointments and transportation as needed.
Provide nursing leadership to unit personnel.
Provide direct care skills (colostomy changes, tube feedings, wound care, suctioning, IV administration, etc.) per policies.
Conduct rounds to ensure resident needs are met and duties performed.
Collaborate with interdisciplinary team to meet residents’ needs holistically.
Notify appropriate departments of concerns or complaints; file grievances as necessary.
Report incidents or unusual occurrences to supervisor, unit manager, assistant director, or director of nursing; assist in investigations.
Participate in end-of-life care as required.
Maintain adequate inventory of medications, supplies, equipment and notify personnel of needs.
Initiate, review and update care plans as needed.
Promote teamwork, mutual respect, and effective communication.
Participate in survey processes when inspection surveys arise.
