Responsibilities

Be a Pillar of Support

– Conduct thorough assessments, ensuring each patient receives personalized, comprehensive care.

Empower Through Education

– Guide and educate patients and their families on health maintenance and disease prevention.

Compassionate Assistance

– Provide gentle assistance with Activities of Daily Living (ADLs) as needed, always with dignity and respect.

Harmonize Care

– Coordinate your schedule directly with patients, aligning with their plan of care and personal preferences.

Vigilant Advocate

– Monitor and promptly report any changes in patient symptoms or behavior.

Healing Hands

– Administer medications and treatments with precision and care, as prescribed by their treating physician.

Collaborative Spirit

– Communicate seamlessly with Case Managers and collaborating physicians or specialists.

Diligent Documentation

– Maintain accurate patient logs in our EMR system.

Emotional Connection

– Offer invaluable advice and heartfelt emotional support to patients and their families.

Communication

– Prepare and provide clear, compassionate clinical notes and charting that tell the patient’s story.

Qualifications

Valid and active RN license in the state of practice.

Willingness to travel to patients’ homes.

Graduated from an accredited nursing school.

Current Basic Life Support (BLS) certification.

Ability to pass a state and federal background check.

Malpractice insurance is required.

Strong problem‑solving and critical thinking skills with a warm, empathetic approach.

Compensation USD $38.00 - $44.00 per hour.
