Recruiter: Kate Davies. The Case Management and Social Work team at UVM Medical Center is seeking a Case Manager II. This position will work with our Inpatient Pediatric team. Please Note: This is an onsite position at UVM Medical Center in Burlington, VT. Responsibilities
Coordinate the care and services of selected patient populations across the continuum of illness; promote effective utilization and monitoring of health care resources; and collaborate with an interdisciplinary team to achieve patient-centered, clinical, and resource outcomes. Identify and address barriers to treatment, health outcomes, and discharge planning, including biopsychosocial and other high-risk factors. Serve as an essential link between the patient, the healthcare team, the payer, and the community. Clinical Competencies & Responsibilities
Clinical Expertise & Judgment
— Apply evidence-based case management practices; conduct comprehensive assessments including medical, psychosocial, and environmental factors; formulate and implement individualized care plans in collaboration with patients, families, and care teams. Communication & Collaboration
— Demonstrate strong verbal and written communication skills; build respectful relationships with patients, families, and interdisciplinary teams; facilitate care conferences and communicate effectively with payers and community partners. Cultural Competency & Inclusion
— Exhibit awareness of personal and cultural values, beliefs, and biases; promote diversity and respect across all interactions; tailor care and education to meet the needs of diverse populations. Regulatory & Compliance Knowledge
— Maintain up-to-date knowledge of HIPAA, JCAHO, CMS, and other regulatory requirements; perform utilization reviews using InterQual or equivalent criteria; issue HINNs and IM notices as appropriate. Teamwork & Professionalism
— Foster a culture of teamwork and mutual respect; demonstrate integrity, compassion, and professionalism; engage in continuous learning and contribute to departmental initiatives. Education & Experience
Education options include: Master's in Social Work from an accredited school of social work (LICSW preferred). Current RN licensure or compact licensure recognized by the State of Vermont (required). Master's degree in a Health or Human Services field (e.g., Counseling); licensure preferred. Bachelor’s of Science or higher in an Accredited Occupational Therapy Program; licensed or eligible to practice OT in Vermont. Bachelor’s of Science or higher in an Accredited Physical Therapy Program; licensed or eligible to practice PT in Vermont. Case management accreditation (e.g., CCM, ACM, ANCC) preferred. Experience
Minimum of 3 years of clinical experience, including 2 years in a medical/hospital setting. 2 years of case management experience or a similar role preferred. Why UVMMC?
