Our RN will perform delegations for Brookdale of Newberg Monday‑Thursday and for Brookdale Wilsonville on Fridays, providing support for changes in condition, RN delegation, etc. LPN Health & Wellness Directors are in these communities. The RN also assists within the two communities, may complete delegated tasks, covers for HWD vacations and on‑call coverage, and handles various other tasks as needed. We provide 4 weeks of training and onboarding prior to your start. You’ll report to the District Director of Clinical. Responsibilities

Provide assistance to Health and Wellness Directors to manage day‑to‑day healthcare operations and ensure residents’ healthcare needs are met. Ensure residents are treated with respect, dignity, and quality care as needs change. Support multiple communities and coordinate with the District Director of Clinical for scheduling. Work with community HWD and HWC to manage resident health, communicate information to families and associates. Identify communities requiring additional clinical resources and prioritize schedules. Participate in pre‑admission screening of prospective residents and assist in assessment per Nurse Practice Act. Initiate individualized service plans and manage care proactively. Evaluate residents, document changes, notify executive director, physician, and legally responsible parties. Prepare Physician Visit Form, review and update resident charts. Contribute to supervision of community‑based licensed nursing staff, medication technicians/C MAs, and Resident Care Associates. Provide training and education to resident care associates through classroom services and situation‑specific training. Supervise maintenance of resident charts and review documentation. Facilitate continuity of care for residents receiving home health care, hospice services, and other third‑party services. Participate in or lead meetings on resident care issues (e.g., Service Plan meetings). Participate in department quality improvement activities. Ensure a safe environment for associates to reduce Workers’ Compensation claims by evaluating transfer and mobility needs and involving therapy services as indicated. Support other regional requirements as assigned. This job description provides an overview of responsibilities. It is not a comprehensive list. Education required to obtain state nursing license (LPN/LVN or RN). Senior living, post‑acute care, or nurse management experience preferred. Basic typing and computer skills (Microsoft Windows). LPN/LVN or RN license. Applies guidelines to make varied decisions and solve moderately complex problems. Extensive knowledge of functional discipline and thorough understanding of organization and work environment. Physical Demands and Working Conditions

Standing, walking, sitting. Use hands and fingers; reach with hands and arms. Climb or balance. Talk or hear. Taste or smell. Ability to lift up to 50 pounds. Vision. Interaction with co‑workers, residents or vendors. Occasional weekend, evening or night work. On‑call as needed.
