The Healthcare Coordinator will oversee case management activities, coordinate home care services, and ensure effective communication between clients, caregivers, and healthcare professionals. Responsibilities include responding to client inquiries, managing and scheduling care services, maintaining client records, and addressing client needs with compassion and efficiency. The individual will work closely with team members to provide exceptional customer service and ensure compliance with healthcare regulations. Qualifications Proficiency in Case Management and experience in coordinating client care plans Strong Communication and Customer Service skills to effectively interact with clients, families, and team members Knowledge and experience in the Healthcare and Home Care domains Ability to organize, prioritize tasks, and manage time effectively Detail-oriented and able to maintain accurate records and documentation Basic computer skills and comfort with healthcare systems or software Previous experience in a healthcare or home care setting is a plus High school diploma or equivalent; additional certifications or qualifications in healthcare administration are advantageou Seniority level
