Job Summary Positions available servicing Hudson or Passaic County. Join a dedicated team committed to supporting families through the Family Connects NJ program! As an Assistant Nurse Supervisor, you’ll empower Nurse Home Visitors to deliver exceptional postpartum care across Hudson or Passaic County. You’ll ensure program integrity, compliance and model fidelity while fostering professional growth through supervision, reflective practice and staff development. Collaborate with fellow Nurse Supervisors to drive success, enhance service delivery and create meaningful impacts for new parents and their newborns.
Essential Duties
Provide direct supervision and evaluation of up to 4.5 FTE Nurse Home Visitors in Hudson or Passaic counties.
Oversee training and orientation of new nursing staff.
Participate in the required Family Connects International model training, ongoing training webinars, and professional development training.
Perform Quality Assurance assessments and quarterly fidelity field visits with each nurse home visitor under their supervision.
Assist the supervisors in leading Monthly Quality Assurance meetings with internal and external stakeholders.
Assign client caseloads and monitor home visitors’ ability to complete home visits and to complete necessary documentation, following the Family Connects NJ model.
Conduct up to 8 in-person visits per month with families residing in Essex and/or Sussex County.
Lead weekly case conferences with nurse home visitors in fidelity with the Family Connects International model.
Develop and maintain community and provider referral sources for the program, along with nurse home visitors and support staff.
Represent the Family Connects NJ program at community events and present on the program to stakeholders as requested.
Effectively support nurse home visitors in analyzing complex problems and clinical case situations and consult with the Medical Director as needed for clinical guidance.
Participate in community advisory board meetings to assist in coordinating services.
Collaborate with other FCNJ staff members to ensure continued implementation of program processes.
Handles other assignments as requested.
Job Requirements
Bachelor’s Degree in Nursing required
Current NJ RN license (in good standing)
BLS certification
Preferred 3 years of experience providing maternal/child health services in a community setting, home visitation, OB‑GYN nursing, neonatal and/or pediatric nursing, labor and delivery, adult mental health or a closely related field.
Preferred 1 year of program management/supervision experience.
Experience working with diverse populations, communities and organizations.
Excellent verbal and written communication skills
Computer proficiency in MS Office Suite
Highly collaborative styles, skilled at relationship and consensus building
Strong initiative and follow-through with an enthusiastic, positive attitude
Ability to work collaboratively with various agencies and stakeholders
Multi‑tasking skills are a must
